Overview
Full-Time Nurse Practitioner role at Riverside Health Care Facilities Inc. The Attending Nurse Practitioner provides primary care to residents in the Rainycrest Long-Term Care Home and works within the NP scope of practice as defined by the College of Nurses of Ontario. The Attending NP collaborates with residents and families, and with the health care team, to develop, implement and evaluate the resident's plan of care.
The Attending NP also leads and mentors LTCH staff and contributes to research, education, and evidence-based practice to optimize resident and LTCH outcomes.
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice (required).
- Post-graduate education and experience in gerontology (recommended).
- Specialty certification in gerontological nursing (recommended).
- Post-graduate certificate and experience in palliative care (recommended).
- Maintains confidentiality and acts in an ethical manner, consistent with professional expectations, the written agreement between the NP and the LTCH, and LTCH policies and code of conduct.
- Is a member of the primary care team and seeks appropriate consultation as required.
- Engages in ongoing professional development, maintains clinical competence and currency with legislation affecting practice.
- Develops in-depth expertise in care of older persons and leadership.
- Holds professional liability protection.
- Maintains CPR certification designed for health-care professionals and standard first aid; demonstrated strong written and verbal communication skills.
- Demonstrated strong interpersonal skills with the ability to function effectively independently, within a team, with frontline personnel, and management.
- Demonstrated ability to function effectively in a fast-paced healthcare environment with excellent organizational and time management skills; able to multitask and prioritize workload; dependable with an acceptable attendance record.
- Proven ability to perform the essential duties of the position on a regular basis.
- The Attending Nurse Practitioner reports to the Director of Nursing or Administrator and is accountable to the Medical Director for LTCH policies, procedures, and medical-services protocols; serves as a primary care provider to residents and collaborates with the health care team in developing and evaluating the resident's plan of care.
- Provides leadership and mentorship to LTCH staff and leads or participates in research, education, and evidence-based practice initiatives to improve resident, LTCH, and health-system outcomes.
- Provides comprehensive primary care to residents as part of the interdisciplinary health care team.
- Performs person-centered health assessments on admission, annually and as needed (including quarterly medication reviews).
- Conducts person-centered health histories.
- Collaborates with residents to develop person-centered plans of care.
- Engages with residents in regular dialogue about their care plan.
- Utilizes communication and counselling skills:
- Engages residents in dialogue to determine what is important to them for health and quality of life.
- Provides person-focused health education.
- Participates in regular care conferences.
- Manages resident care by providing pharmacological, complementary and/or counselling interventions, and performs procedures within the NP scope of practice.
- Orders and/or performs appropriate screening and diagnostic investigations, interprets results, and assumes responsibility for follow-up.
- Diagnoses acute and chronic health conditions.
- Provides outreach and transitional services to LTCH residents who return to the community.
- Liaises with discharge planning services for hospitalized residents to ensure a smooth transition back to the LTCH.
- Participates in after-hours and on-call coverage in accordance with the LTCH on-call policy.
- Increases continuity of care through collaboration, consultation and referral as appropriate.
- Collaborates and consults with physicians, the director of nursing and personal care, nursing staff, interdisciplinary team members and external resources regarding resident plan of care.
- Assists, supports, guides and provides consultation to director of nursing and personal care, nursing staff and interdisciplinary team members regarding challenging clinical situations.
- Makes referrals to specialized consultants, services and other health providers.
- Accepts referrals from interdisciplinary team members.
- Advocates for and provides palliative and end-of-life care.
- Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses and clinical rationale in a timely and accurate manner.
