Pay: $33-36/hour Schedule: Full-Time | 8am - 5pm, Monday - Friday | Hybrid
Help Drive Recovery and Stability Through Compassionate NursingOur leading behavioral health nonprofit client is seeking a dedicated Licensed Vocational Nurse (LVN) to join their community-based care team. This full-time role supports individuals experiencing mental health challenges, homelessness, and co-occurring conditions by delivering medication support, care coordination, and medical linkage services both in the clinic and out in the field.
As the LVN, you’ll work closely with a psychiatrist and interdisciplinary team to ensure high-quality, person-centered care. If you want a nursing role that blends clinical expertise, fieldwork, and meaningful relationships with clients—this is it.
What You’ll DoDirect Care & Support
- Provide medication support in clinic and community settings
- Administer medications and follow up on side effects or symptom changes
- Coordinate with psychiatrist on treatment planning and medication management
- Conduct medical and behavioral health referrals and follow-ups
- Educate clients on chronic health conditions, treatments, and wellness strategies
- Help reduce hospitalizations and support long-term housing and stability
- Work closely with case managers, therapists, and psychiatrists
- Attend team meetings, clinical rounds, and trainings
- Support clinic days and collaborate across programs to ensure continuity of care
- Provide coverage for nursing teammates as needed
Documentation & Compliance
- Maintain timely, accurate documentation in the electronic health record (EHR)
- Adhere to HIPAA, Medi-Cal, and Medicare standards
- Monitor and maintain medication logs, storage, and temperature tracking
- Uphold regulatory and accreditation standards across nursing protocols
- Active California LVN license (unrestricted)
- 2+ years of experience in behavioral health, acute care, home health, or similar settings
- Valid California driver’s license and reliable transportation (travel required throughout High Desert area)
- Familiarity with co-occurring disorders and community-based service delivery
- Strong documentation skills and comfort using EHR systems
- Ability to work with vulnerable populations in varied environments
- Bilingual (Spanish) preferred, but not required
