Job SummarySigma Inc. is seeking an experienced Licensed Vocational Nurse (LVN) to join our team onsite in Irving, TX as a Care Coordinator supporting CHRISTUS Health's value-based care initiatives. In this role, you will help improve patient outcomes by conducting telephonic outreach, identifying care gaps, coordinating preventive services, and supporting providers across ACO & CIN networks. This is an excellent opportunity for LVNs with strong patient communication and quality-care experience.
Key Responsibilities- Conduct telephonic outreach to attributed members to close care gaps and support preventive care.
- Identify and address quality gaps using value-based care performance metrics.
- Support Primary Care Providers by helping patients schedule screenings and follow-up appointments.
- Communicate with healthcare providers via system tools, meetings, and secure messaging.
- Pull and review reports to monitor quality performance and medication adherence.
- Provide health education, counseling, and standardized patient instructions.
- Serve as a subject matter expert in care transitions and quality metrics.
- Participate in Quality Improvement Programs as needed.
- Document patient interactions and follow internal protocols for compliance.

Competencies:
Leader of Self, Leader of Others, Leader of Leaders.
- Active Licensed Vocational Nurse (LVN) license – Must reside in Texas.
- Nursys license verification required with submission.
- Minimum 3 years of clinical OR home health experience.
- High School Diploma or GED (must submit proof of education).
- Strong communication skills for telephonic outreach and patient interaction.
- Experience with value-based care, care coordination, or quality improvement preferred.
- Knowledge of ACO / CIN models
- Experience with medication adherence programs
- Familiarity with quality measures (HEDIS, Star Ratings, Preventive Care Metrics)
- EMR / EHR documentation experience
