What you’ll bring as Clinical Team Manager
  • Provide oversight of all patient care services and personnel, including assignments, coordination of care, referrals, and development of individualized patient care plans.
  • Plan, direct, and participate in delivery of home health services to provide diagnosis‑specific patient care, coordinating services with providers, vendors, or facilities.
  • Ensure the timely start of patient care within recommended regulatory guidelines.
  • Process and coordinate physician orders, staff assignments, and communications among direct‑care team members, patients, families, and physicians.
  • Review and approve plans of care and evaluate proposed changes for clinical appropriateness.
  • Integrate orders from all relevant physicians and ensure they are approved by the responsible physician.
  • Conduct regular OASIS reviews for all patients.
  • Meet regularly with direct‑care team members, provide guidance, coaching, and communicate information related to specific issues or organizational changes.
  • Initiate hiring requests, interview candidates, and recommend hiring of direct‑care staff.
  • Communicate frequently with the Branch Director or Administrator on key performance metrics.
  • Remain available for field work as needed.
What is required
  • Associate degree in Nursing or related field;
    Bachelor preferred.
  • 2 years of home‑care experience in a Home Health or Hospice environment.
  • 1 year of supervisory experience.
  • Current, unrestricted RN license valid for the state of work.
  • Proficiency in enterprise applications such as Homecare Homebase, Workday, and Microsoft Office.
  • Able and willing to travel up to 50% as needed.
  • Physical ability to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.
  • Valid driver’s license, insurance, and reliable transportation.

You will report to the Administration or Branch Director.
