Registered Nurse Navigator Population Health Senior - Population Health Admin
CHRISTUS Health seeks a Registered Nurse Navigator Population Health Senior to coordinate and manage a designated patient caseload across the healthcare continuum, improving health outcomes through evidence‑based practices and preventive care.
Responsibilities- Meet applicable OneCHRISTUS Competencies (Leader of Self, Leader of Others, or Leader of Leaders).
- Mentor, train, and support a team of nurse case managers.
- Manage and coordinate care for patients with complex medical needs and those at high risk for hospitalization.
- Conduct comprehensive assessments to identify patient needs, barriers to care, and social determinants of health.
- Develop and implement individualized care plans based on assessments, clinical guidelines, and patient preferences.
- Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle choices.
- Track patient progress, adjust care plans as needed, and evaluate outcomes.
- Foster a collaborative environment with providers, social workers, and community resources for a holistic approach.
- Collaborate with leadership on market‑specific strategies, data analytics, and action plans to reduce acute and post‑care utilization.
- Lead quality improvement initiatives with the team of nurses to enhance patient care and population health outcomes.
- Ensure compliance with all regulatory requirements, organizational policies, and best practices in case management.
- Coordinate associate onboarding and provide ongoing training and professional development opportunities.
- Assist with special projects as needed.
- Perform other duties as assigned.
- Bachelor's degree in nursing (BSN) required.
- 3 years of clinical nursing experience.
- 5 years of case management experience.
- Experience working in primary care value‑based care organizations.
- Strong clinical assessment skills.
- Excellent communication and interpersonal skills.
- Proficiency in keyboarding, EHR systems, and population health management principles.
- RN license in the state of employment or compact.
- Required certifications:
- Certified Case Manager (CCM) by CCMC
- Nursing Case Management Certification (CMGT‑BC) by ANCC
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all associates are required to obtain the required certifications for their respective positions within the designated time frame.
Work Schedule8 AM – 5 PM Monday‑Friday
Work TypeFull Time
Dallas, TX – Salary $85,000 – $128,000 per year.
