Licensed Vocational Nurse (LVN) - Full-Time/Per Visit - Home Health

We are seeking a skilled and compassionate Home Health LVN to join our team. The Home Health LVN will be responsible for providing nursing care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in home health care, excellent communication skills, and a passion for helping others.

• Assess patients’ health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients’ vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures

• Current LVN license in the state of Texas
• Minimum of one year of experience in home health care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team Strong critical thinking and problem‑solving skills
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• Wound care 2-3 x week
