As a Licensed Nurse RAI Coordinator, you will be responsible for:

• Conducting and facilitating RAI assessments for residents
• Coordinating with healthcare teams to ensure comprehensive assessments
• Maintaining accurate and up-to-date records
• Identifying and addressing resident needs and concerns
• Collaborating with other departments to ensure seamless care delivery
• Providing exceptional customer service to residents, families, and healthcare providers

• Conduct and facilitate RAI assessments for residents, ensuring thorough and accurate documentation
• Work closely with the interdisciplinary care team to identify and address resident needs and concerns
• Develop and maintain accurate and comprehensive resident records
• Identify and report potential issues or concerns to the care team
• Collaborate with other departments to ensure seamless care delivery and communication
• Provide education and support to families and healthcare providers as needed
