Job Description

This RN/LPN positon will play a vital role in delivering compassionate and culturally sensitive nursing care to patients in a home health setting. This position requires proficiency in Spanish to effectively communicate with patients and their families, ensuring high-quality care and understanding throughout the treatment process.

Responsibilities

• Provide skilled nursing care to patients in their homes, tailored to individual treatment plans
• Monitor patient health and document care provided accurately
• Implement and manage bowel programs for patients as needed
• Educate patients and family members on managing health conditions and medications
• Coordinate with healthcare providers to support continuity of care
• Advocate for patient needs and cultural considerations during care delivery

Required Qualifications

• Spanish language proficiency

• Current LPN or RN License
