To provide a specialist clinical pharmacy service to patients within the Emergency and Integrated Care Division (EIC). To ensure the safe, effective, legal and accurate use of medicines within the speciality and to support and supervise less experienced pharmacists on rotation. To act as a role model and to deliver, develop and evaluate specialist clinical pharmacy services in accordance with the objectives set by the line manager.

Working within the medicine directorate covering wards and dispensary as a clinical Pharmacist.

Working on‑call weekday and weekends with additional weekend shifts as part of a rota system.

Job responsibilities

To provide a clinical ward pharmacy service to wards in the medicines EIC division. To support the development and implementations of medicine related guidance, policies and procedures within a speciality. To support the integration of clinical pharmacy services within the MDT and within strategic decision‑making. To undertake medicines reviews to reduce inappropriate polypharmacy and adverse effects and to improve adherence and patients understanding of medicines.

To screen home‑care prescriptions for safety and appropriateness. Work with home‑care companies to ensure a smooth service for patients that meets the needs of the patients and the Trust. Answer all home‑care queries. Audit the home‑care services and implement measures to improve the service and reduce waste. Counselling of patients newly started on oral anticoagulants Medicines usage review where appropriate Supporting the Out‑Patient Antibiotic Therapy (OPAT) Service.

Facilitating the discharge process Weekly AEC MDT meeting attendance Supporting the supply of medications to the department, particularly biologics, intravenous antibiotics and Low Molecular Weight Heparins (LMWHs) To support the development and implementations of medicine related guidance, policies and procedures within the speciality. Work closely with Medicines Management Technicians (MMTs) and supervises junior pharmacy staff as directed by the Lead Directorate Pharmacist /Highly Specialist Pharmacist.

To support the EIC divisional pharmacist in the development and implementations of medicine related guidance, policies and procedures within the speciality. To undertake a quality improvement project within the division. To support with the development and implementation of shared‑care guidelines to promote seamless care for patients. To gain an insight on the drugs budget and drug expenditure within the division.
