Resettlement Support Worker

We're looking for a compassionate and proactive Resettlement Officer to support people leaving hospital who may be homeless, seeking asylum, or facing significant barriers to housing and stability.

Based in a small, short-stay accommodation service in Stoke Newington, you'll play a vital role in helping customers settle into the community, develop essential life skills, and move on to longer-term housing. This is a varied and rewarding role where you'll build trusted relationships and make a meaningful difference to people's lives every day.

What you’ll do

You'll provide high-quality housing management and resettlement support, helping customers gain the confidence and skills needed to live independently. Your responsibilities will include:

* Supporting customers to settle into their accommodation and help them access benefits, healthcare, community services and other sources of support.

* Providing practical guidance on independent living, including budgeting, money management, healthy eating and maintaining a home.

* Work closely with hospitals, housing providers, support agencies and community organisations to achieve positive outcomes.

* Supporting customers to identify and move on to suitable long-term accommodation.

* Carrying out health and safety checks, room inspections and welfare visits to maintain a safe and welcoming environment.

* Responding to safeguarding concerns and working proactively to manage risk.

* Maintaining accurate records, support plans and risk assessments.

About You

We're looking for someone who is passionate about helping vulnerable people rebuild their lives and achieve greater independence. You'll be patient, resilient and a confident communicator who can build positive relationships with customers and partner agencies. You’re flexible and adaptable, with the ability to respond to a wide range of customer needs, but also comfortable working independently and making decisions when lone working.

You’ll also have:

* Experience supporting vulnerable adults within housing, homelessness, resettlement, supported housing or a similar environment.

* Knowledge of housing management, welfare benefits and tenancy sustainment.

* An understanding of safeguarding, health and safety and risk management.

* Experience completing support plans, risk assessments and customer records.

* Knowledge of recovery-focused and/or trauma-informed approaches.

* The ability to support customers with practical life skills and independent living.

* Experience working collaboratively with external agencies and stakeholders.

* Good IT skills, including Microsoft Office and electronic case management systems.

* Organised, with strong report-writing and record-keeping skills.

This is a 24-hour resettlement service supporting up to six customers transitioning from hospital into the community. You'll work on a rota basis covering shifts between: 8:00am to 4:00pm & 1:00pm to 9:00pm. The service operates Monday to Sunday, and the role includes lone working with management on-call support available when required.

This role will require an Enhanced DBS check and as an employer, Peabody does not provide sponsorship as a licenced UK employer.
