LPN Med Care Manager

Role: Licensed Practical Nurse (LPN) Medication Nurse

• Administer medications and treatments accurately according to physician orders
• Follow the Six Rights of Medication Administration to ensure safety
• Document all medication activities in MAR/TAR in real time
• Monitor resident symptoms, administer medications for relief, and report adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs) and support care plan updates through transcription of physician orders
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, perform narcotic counts, and ensure regulatory compliance

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and clinical documentation skills
• Experience in senior care or medication administration preferred
