Memory Care Program Coordinator

We are seeking a compassionate and organized Memory Care Program Coordinator to join our team at Landmark of Itasca. As a Memory Care Program Coordinator, you will play a vital role in ensuring the highest quality of care for our residents living with Alzheimer's disease and other memory-related conditions.

In this role, you will be responsible for coordinating the daily programs and activities for our memory care residents. This will include developing and implementing activity plans, conducting assessments to determine resident interests and abilities, and collaborating with the care team to ensure seamless care delivery. You will also be responsible for maintaining accurate records, reporting, and statistical analysis to support program evaluation and improvement.

• Develop and implement activity plans tailored to the needs and interests of our memory care residents

• Conduct assessments to determine resident abilities, interests, and needs

• Collaborate with the care team to ensure seamless care delivery and support resident care goals

• Maintain accurate records, reports, and statistical analysis to support program evaluation and improvement

• Coordinate daily programs and activities, including arranging transportation, meals, and other logistical details

• Provide support and guidance to caregivers and other team members

• Participate in resident care conferences and other inter-shift meetings to promote communication and collaboration
