Our interdisciplinary team works closely with physicians to deliver care after injury, surgery, or throughout chronic illness management.
Services include home health care (aides, nursing, therapy, medical social work), remote monitoring for chronic conditions, and access to 24/7 home medical equipment support. We also offer advanced palliative care through our AIM program, as well as hospice care focused on maximizing comfort and quality of life.

General SummaryThe Home Care Occupational Therapist is responsible for the delivery of patient care through the process of assessment, development, implementation, education and revision of treatment programs as necessary for provision of age-specific direct care/services to patients.
Education & Experience- Graduate of an accredited occupational therapist program;
Bachelors or Master's degree required.
- Licensure in the State of Michigan as an Occupational Therapist required.
- Requires valid Michigan driver's license and dependable vehicle insured as required by the State of Michigan.
