Position

Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence, per physician or advanced practice practitioner orders and plan of care under the supervision of the Home Health leadership team.

Responsibilities
  • Evaluate patient’s functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
  • Evaluate home environment for hazards and barriers to more independent living
  • Participate in development of the individualized plan of care including equipment needs
  • Develop treatment program and establish goals for improved function
  • Communicate plan of care to referring physician and other organization professionals
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with the agency policy
  • Maintain appropriate clinical records, progress notes, and reports to the physician or advanced practice practitioner of any changes in the patient’s condition
  • Submit records in accordance with the agency policy
Required Qualifications
  • Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the AMA and the American Occupational Therapy Association
  • 1-year clinical experience as an Occupational Therapist preferred
  • Registered with the National Registration Examination of the American Occupational Therapy Association
  • Interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregivers
  • Knowledge of medical terminology
  • Strong problem-solving, organizational skills, attention to detail, and ability to handle multiple priorities
  • Strong verbal and written communication skills
  • Reliable transportation and valid automobile insurance coverage
  • Sit, stand, bend, lift, and move intermittently; ability to lift at least 35 lbs
Desired Qualifications
  • Home Health experience preferred
  • Familiarity with Home Care Home Base (HCHB)

$75,000—$90,000 USD
