Through our Healthcare Partnership Program (HPP), we embed clinicians directly within partner organizations to provide accessible, proactive, and preventative care for employees and their families. Our model emphasizes relationship-based care, early intervention, and long-term well-being.

We are seeking a Nurse Practitioner to join our Healthcare Partnership Program (HPP). In this role, you will be placed onsite at a partner organization to provide high-quality primary and preventive care to employees, spouses, and dependents.

This position is ideal for a clinician who enjoys working autonomously while remaining supported by a collaborative healthcare team. You’ll focus on proactive health management, chronic condition support, and addressing urgent health needs — all within a workplace setting designed to remove barriers to care.

• Starting at $50/hour, based on experience
• 8-hour shift (8:00 AM – 4:00 PM)
• Tuesday or Wednesday (set schedule)

• Provide advanced clinical care including acute/episodic care, preventive services, and chronic disease management
• Deliver care to HPP and school-based patients as applicable
• Collaborate with HPP nurses on claims analysis and care planning
• Establish and follow clinical pathways and treatment protocols
• Conduct telehealth visits for HPP patients as needed
• Coordinate care with patients’ external medical providers and networks
• Serve as a clinical consultant while maintaining high standards of care
• Maintain all required licensure and certifications

Required Qualifications:

• Master’s degree in Nursing from an accredited program
• Active, unrestricted State of Michigan Registered Nurse License
• Active, unrestricted Michigan Nurse Practitioner Specialty Certification
• Active FNP national board certification through:
• American Academy of Nurse Practitioners (AANP), or
• American Nurses Credentialing Center (ANCC)
• Minimum 2 years of nursing experience in applicable settings (preferred)
• BLS Certification from AHA or Red Cross (or obtained within 90 days of hire)

Preferred Qualifications:

• DOT Examiner Certification (training available if needed)
• Additional certifications or experience in areas such as:
• Primary Care
• Health Coaching
• Case Management
• Women’s or Men’s Health
• Sports Medicine
• Hospital-based specialties (Med-Surg, ICU, ED)

• Ability to communicate verbally and clearly
• Fully functional vision (with corrective lenses if needed)
• Ability to sit, stand, and walk for extended periods
• Ability to lift or carry up to 25 pounds occasionally
• Ability to operate standard clinical and office equipment
• Reliable transportation for occasional travel to partner locations
• Physical requirements may change based on business needs
