Gentiva Hospice in Jackson, MI is seeking an experienced Hospice Clinical Liaison to coordinate intake and referrals, assess patient eligibility, and support admissions across hospitals, home health agencies, physicians, and case managers.
You will educate patients and families about hospice care, ensure documentation and payer requirements are met, and serve as a liaison between referral sources and our care team to enable timely, compassionate service.
