• Collaborate with the Regional Practice Manager within the Practice Dyad to oversee providers in the Practice.
• Serve as an expert in Physician Services EMR, internal operating systems, and resources to achieve quadruple aim outcomes.
• Assist the Practice in delivering services where patients, families, referral sources, and payors expect exceptional care.
• Provide primary and/or specialty medical care in various settings (assisted living, home, sub-acute rehabilitation, long-term care).
• Develop orientation plans for newly hired providers and oversee their orientation with the Regional Practice Manager.
• Support facility relationship management to grow the practice or aid in service recovery when needed.
• Advance Practice Practitioners within scope of the collaborative/Practice Agreement and ensure timely implementation of organizational initiatives.

• State certification and degree to practice medicine as a Nurse Practitioner.
• Current State Nurse Practitioner License.
• 2 years of clinical experience in a post-acute care setting.
• Ability to practice medicine as defined by licensure boards for Nurse Practitioners.
• Ability to assess and respond to the needs of patients, families, and caregivers in post-acute care settings.
• Demonstrated skill in medical practice, with ability to handle family emotional stress and diverse patient lifestyles.
• Ability to manage multiple priorities, documentation requirements, and deadlines.

• Management experience (preferred).
