Key Responsibilities- Collaborate with the Regional Practice Manager within the Practice Dyad to oversee providers in the Practice.
- Serve as an expert in Physician Services EMR, internal operating systems, and resources to achieve quadruple aim outcomes.
- Assist the Practice in delivering services where patients, families, referral sources, and payors expect exceptional care.
- Provide primary and/or specialty medical care in various settings (assisted living, home, sub-acute rehabilitation, long-term care).
- Develop orientation plans for newly hired providers and oversee their orientation with the Regional Practice Manager.
- Support facility relationship management to grow the practice or aid in service recovery when needed.
- Advance Practice Practitioners within scope of the collaborative/Practice Agreement and ensure timely implementation of organizational initiatives.
- State certification and degree to practice medicine as a Nurse Practitioner.
- Current State Nurse Practitioner License.
- 2 years of clinical experience in a post-acute care setting.
- Ability to practice medicine as defined by licensure boards for Nurse Practitioners.
- Ability to assess and respond to the needs of patients, families, and caregivers in post-acute care settings.
- Demonstrated skill in medical practice, with ability to handle family emotional stress and diverse patient lifestyles.
- Ability to manage multiple priorities, documentation requirements, and deadlines.
- Management experience (preferred).

- Main Service Area: dependent on hire location
- Office hours: 8:00 AM – 5:00 PM, Monday through Friday
