• Planning, directing, and participating in the delivery of professional services to hospice patients and families
• Communicating significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient care
• Initiating a plan for patient safety, using patient, family, and community resources
• Identifying deviations from normal nursing and normal patient condition status
• Directing all aspects of operations and care delivery in accordance with the mission and values of Optimal Care Hospice
• Supervising coordination of services with providers, vendors, or facilities in the delivery of care and related issues
• Processing and coordinating patient referrals, physician orders, staff assignments, and patient re‑certifications, and communicating between the referring physician, various caregivers, the patient and their family
• Supervising and providing clinical direction to RN, LPN, chaplain, MSW, aide, music therapist, massage therapist, volunteers, and bereavement coordinator to ensure quality and continuity of services provided
• Meeting with all field teams regularly to provide guidance and information
• Maintaining adequate staffing levels to meet patient needs
• Coordinating services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referral
• Coordinating weekend on‑call services for the field team

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• 2–3 years experience as a Registered Nurse
• Nursing skills as defined by accepted nursing standards
• Current and valid driver’s license
• Reliable transportation and valid automobile insurance coverage
• Knowledge of business management, governmental regulations, and CHAP standards
• Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
• Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines

• BSN preferred
• Home Health or Hospice experience preferred
