- Provide hands‑on, compassionate care following each patient’s care plan

- Assist patients and families in learning self‑care skills
- Report updates to the RN Case Manager and care team
- Document care provided, changes in condition, and patient needs
- Help prevent unnecessary hospitalizations and support continuity of care
- High School Diploma or GED
- 1+ year of experience as a Nursing Assistant or Hospice Aide
- Completion of a state‑approved Hospice Aide training program with competency evaluation (per 42 CFR 484.36)
- Hospice or home health experience preferred
- Reliable transportation, valid driver’s license, and auto insurance (50% travel)
- Preferred to live in the service area
- Ability to lift 50–100 lbs and perform hands‑on caregiving duties
