Base pay range: $33.40/hr – $43.14/hr

Position Summary

The Licensed Practical Nurse is responsible for providing high‑quality nursing care to residents, coordinating total nursing care, and ensuring compliance with all policies, Michigan Department of Health regulations, and federal Health Care Administration regulations.

Responsibilities
  • Assess resident conditions, implement interventions, and develop Care Plans.
  • Record resident observations accurately in PCC and execute Care Plans.
  • Conduct regular rounds with physicians to observe and evaluate resident progress and responses to treatments.
  • Provide preventative, supportive, maintenance, and rehabilitative care for residents’ physiological and psychosocial needs.
  • Perform treatments such as dressing changes, wound cleansing, Foley catheter irrigations, IV therapy, tube feedings, chest tubes, and peritoneal dialysis.
  • Ensure supplies and medical equipment are available for resident care.
  • Schedule appointments, physicians, therapy, and consults.
  • Document appointments in the appointment book.
  • Assist CNAs with personal resident care duties (bathing, feeding, ambulation, etc.).
  • Review outgoing nurse reports and conduct daily rounds.
  • Administer medications according to the Public Health Code, Nursing policies, and physician orders.
  • Advise residents or responsible persons regarding medication changes, treatment plans, and resident condition.
  • Manage emergency care, coordinate communication with physicians, the DON, administrator, hospital personnel, and emergency transport.
  • Perform comprehensive nursing assessments; admit, transfer, and discharge residents.
  • Assist in the completion of weekly, periodic, and discharge summaries.
  • Participate in quality improvement activities and monitor accident/incidence and infection rates.
  • Perform weekly skin assessments for pressure injuries and report findings to the DON.
  • Report complaints, unusual occurrences, or any allegations of abuse/neglect immediately to the administrator.
  • Participate in periodic regulatory surveys and prepare plan‑of‑correction responses.
  • Report resident injuries to the unit manager.
  • Know residents’ rights, report abuse or violations promptly.
  • Support facility philosophy, standards, and policies.
  • Communicate with department heads as needed to solve problems and plan effective patient care.
  • Inform the DON of staffing shortages.
  • Maintain a professional appearance and serve as a unit role model.
  • Continuously pursue education and development through seminars and related training.
  • Promote safety procedures and safe work practices to prevent employee injury and reduce worker’s compensation costs.
  • Use equipment safely and report maintenance concerns to the Environment Plant Manager.
  • Adhere to the Nurse Practice Act and attend staff meetings and in‑services as required.
  • Perform other tasks as required.
Qualifications Education, Training, and Experience
  • Current Michigan license as a Licensed Practical Nurse.
  • CPR certification.
  • Preferably two years of professional nursing experience in a long‑term care setting.
Specific Skills, Knowledge, and Abilities
  • Knowledge of federal and state laws and regulations applicable to long‑term care nursing.
  • Strong organizational and time‑management skills with the ability to prioritize responsibilities.
  • Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Basic computer proficiency.

Special Requirements
  • Tolerate frequent exposure to blood, body tissues, fluids, hazardous materials, infectious diseases, chemicals, latex, and PPE plastics.
  • Manual dexterity to operate modern office equipment.
  • Ability to travel as needed or assigned.
