The Home Care Coordinator, LPN provides direct and indirect nursing care to Program of All-Inclusive Care for the Elderly (PACE) participants in the home care setting. Working under the supervision of the Community RN Supervisor or designee, this role uses sound clinical judgment and advanced nursing skills to manage participant care, coordinate services, assess ongoing needs, and monitor health status. The Home Care Coordinator, LPN collaborates closely with physicians, nurses, and interdisciplinary team members to identify changes in condition, address emerging health concerns, and ensure high-quality, participant-centered care.
Assessment& Care Planning
- Conduct in-home assessments and collect participant data per PACE standards.
- Identify participant needs, set priorities, and make referrals as appropriate.
- Participate in interdisciplinary team (IDT) meetings and contribute to care planning.
- Develop and revise comprehensive care plans with input from participants, families, and the care team.
- Provide direct nursing care, including treatments, medication education, IV therapy, tube feeding, and wound care.
- Conduct home visits and coordinate care across nursing homes, assisted living, and foster care settings.
- Monitor participant progress, document in EMR, and communicate changes to the care team.
- Assist with ordering and coordinating durable medical equipment (DME) and supplies.
- Conduct monthly in-home supervision visits to ensure care plans are followed.
- Support quality improvement initiatives and on-call nursing needs.
- Delegate care appropriately and maintain safe, efficient workflow.
- Apply cost-effective measures and adhere to organizational policies.
- Address care management issues and participate in problem resolution.
- Graduate of an accredited School of Nursing.
- Licensed Practical Nurse (LPN) in the State of Michigan.
- Must have reliable transportation and possess a valid and current driver’s license.
- Must be CPR certified or willing to obtain it.
- Minimum of three (3) years of experience as a practicing nurse in a health-related area.
- At least one (1) year of experience working with the frail and elderly, particularly in a homecare or community setting.
- Experience in managing or coordinating patient care, especially for individuals with complex health needs.
- Knowledge of or experience in working within a PACE (Program of All-Inclusive Care for the Elderly) program or similar.
- Elderly care models is a plus.
- Ability to effectively communicate with patients, families, and interdisciplinary teams.
- Strong critical thinking and problem‑solving skills, with the ability to work independently and manage complex clinical situations.
