Make a Difference with Center Well Senior Primary Care Do you want to practice medicine the way it was meant to be? Join our team of passionate providers at Center Well Senior Primary Care and build meaningful relationships with patients while working collaboratively with our care teams.
As a Geriatric, Family Medicine, or Internal Medicine Physician, you'll have the opportunity to:
-Spend quality time with patients, digging into their health stories and truly understanding their needs inside and outside the exam room.
-Take a hands-on, team-based approach to care. Work closely with nurses, social workers, pharmacists, and other providers to develop comprehensive care plans tailored to each patient.
-Thrive in our unique value-based care model that prioritizes quality of life and improved outcomes for seniors. We go beyond the clinic walls to address social determinants of health.
-Enjoy strong physician leadership, mentoring, and a supportive work culture.
The role:
-Full-time outpatient only, no weekends or on-call-Patient panel of 12-15 per day-Emphasis on preventive care, chronic disease management, care coordination-Opportunity for leadership and development
