Epic required (uses navi Health for referrals; moving toward Epic Care Management)
Experience
Required:
2+ years acute care case management/discharge planning, including 6+ months in a Level I or large academic medical center
Schedule:
5x8s M–F 8:00a–4:30p (non-negotiable) | Weekends:
None | Holidays:
Rotated as needed
Caseload: ~15–20 patients/day
Certs/
Education:
BLS + RN (CCM preferred); BSN preferred (ADN considered)
Traveler Rules: 50-mile radius; locals accepted (local rate applies)
Requirements:
Flu vaccine required (medical exemption only w/ documentation) + fingerprinting (allow ~10 days remote or in-person the week prior)
Orientation: ~2 weeks with team support (not working alone)
