Position Overview:
Human Edge Health is seeking an experienced Travel RN Case Manager to support acute care discharge planning in Jackson, Mississippi. This role covers all inpatient units within a large Level I trauma and academic medical center and requires strong experience in complex medical discharge planning.

Key Responsibilities:

• Provide RN Case Management with a focus on discharge planning

• Manage a caseload of approximately 15–20 patients per day

• Coordinate medical discharges, including some social discharges

• Collaborate closely with physicians, social work, pharmacy, respiratory therapy, and interdisciplinary teams

• Assess discharge readiness and identify appropriate post-acute care placements

• Utilize EMR systems to document care and submit referrals

• Support complex placements, including uninsured and underinsured patient populations

Requirements:

• Active RN license (willing to wait for licensure)

• BLS certification

• Minimum 2 years of acute care RN Case Management experience

• At least 6 months of experience in a Level I Trauma Center or large academic medical center

• Strong discharge planning experience across multiple inpatient units

• EPIC charting experience (required)

• CCM certification preferred but not required

• ADN considered; BSN preferred

• EMR: EPIC (navi Health currently used for referrals; transitioning to EPIC Care Management)

• Works alongside a robust Case Management and Social Work team

• Reports to the Social Work Manager with oversight from the Chief of Care Coordination
