• Assess Patient Needs: Identify and evaluate the physical, psychological, social, and spiritual needs of patients and their families.
• Develop Care Plans: Initiate appropriate interventions and create comprehensive, responsive care plans tailored to each patient's unique needs.
• Coordinate Care: Collaborate with the interdisciplinary team (IDT) to ensure seamless delivery of services to patients and families.
• Educate Caregivers: Provide education to patients, families, and primary caregivers on effective care techniques and best practices.
• Advocate for Patients: Maintain regular communication with attending physicians, long‑term care facility staff, HMO case managers, and other stakeholders to advocate for the patient's needs and preferences.

• Licensure: Valid New Jersey Registered Nurse (RN) license.
• Experience: Minimum of two (2) years of nursing experience in an acute‑care hospital setting, preferably in medical, surgical, or oncology units.
