• Monitor clients’ conditions
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments
• Case management and coordination
• Accurately document observations, interventions, and evaluations
• Travel to clients in local area on a schedule that fits your needs

• Current New Jersey nursing license in good standing
• Graduation from an accredited, approved nursing program (transcript or diploma)

Base pay ranges from $55.00‑$62.00 per point, depending on qualifications and subject to negotiation.
