HHA schedule: 20-30 hours on Weekdays (daytime) immediately, with additional hours available in evenings and weekends (if desired).

Job Summary

We are seeking a skilled, compassionate, and professional Home Health Aide (HHA) / In-Home Caregiver to join our home health agency. HHAs / Caregivers at HCN are professional, compassionate, and reliable, delivering critical care and living services to our clients which empower their independence and provide comfort, stability, and peace of mind to them and their families.

Compensation

Hourly pay ranges from $12.50 - $14.00. Compensation is based on qualifications, experience, positive employment history, and/or certifications.

Qualifications
  • 1+ year of verifiable experience in home care or caregiving setting is required.
  • Successful completion of a formal certification training program (CNA, STNA, CHHA) is preferred, and/or a written skills test and competency evaluation. HCN will train non-certified caregivers committed to building a career with our agency.
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.
  • CPR and First Aid Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Strong attendance and dependability track record with previous employers.
  • Good interpersonal skills and compassion for helping others live their best lives are required.
  • Demonstrated ability to read, write, and follow a written Plan of Care, documenting notes and communicating with our agency.
  • Desired character traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly and accommodating, respectful.
  • Be at least eighteen (18) years of age.
  • Valid driver's license/ reliable transportation to/from client care locations is mandatory.
Reports to

RN Case Manager / Administrator

Responsibilities
  • Deliver direct patient care and support, following the established plan of care.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail and skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assists patients in taking medications under the direction and supervision of the Registered Nurse (this includes opening a container or pill caddy and handing it to the patient, but does not include administration of medications).
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
Additional Essential Functions and Areas of Accountability
  • Know and follow infection control policies including proper hand washing techniques.
  • Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Document care provided and complete all forms required for the client's records.
  • Promote personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
