PRN PHYSICAL THERAPIST (PT)

Location: Jackson, TN

Facilities: Maplewood Healthcare & Laurelwood Healthcare

About the Facilities

Join a supportive in-house rehab team dedicated to exceptional patient outcomes and meaningful collaboration.

Responsibilities
  • Review available resident information and evaluate the impact on assessment and treatment
  • Conduct and document thorough functional resident assessments using standardized procedures
  • Develop and implement individualized treatment plans with achievable functional goals
  • Comply with evaluation, treatment, and documentation guidelines; maintain timely and accurate records
  • Instruct Assistants in treatment procedures and provide supervision while they work with residents
  • Enter treatment and progress notes after each session
  • Revise treatment plans as needed based on resident performance
  • Document the course of treatment in clear, concise discharge summaries
  • Orient residents to program services and treatment procedures appropriate to their needs
  • Collaborate with nursing staff, residents, and families to maximize functional abilities
  • Educate residents and caregivers on therapeutic exercises and procedures for home continuation
  • Assist with referral processes for additional services post‑discharge
  • Attend and participate in rehabilitation team conferences and other facility meetings
  • Access continuing education opportunities to maintain clinical competency
  • Participate in quality improvement activities
  • Adhere to Ahava Therapy Services’ regulatory and professional standards
  • Follow facility policies and participate in safety programs
  • Perform other duties as assigned
  • Understand reimbursement guidelines and allocate therapy resources efficiently
  • Maintain productivity levels and support the company mission and vision
Qualifications
  • Bachelor’s or Master’s degree in Physical Therapy
  • Current Tennessee PT license
  • Current BLS/CPR certification
  • Strong observational, verbal, and written communication skills
  • Ability to work collaboratively with an interdisciplinary team
