As a Primary Care Provider, your responsibilities will include delivering comprehensive primary care services with an emphasis on improving access and outcomes in underserved areas, functioning within a progressive clinical model that features advanced scheduling and strong clinical support, and collaborating within a multidisciplinary team to provide personalized and community-based health services.andlt;/pandgt; andlt;pandgt;

Eligible candidates include physicians with an MD/DO degree from an accredited medical or osteopathic institution, completion of a family practice residency program, and board certification or eligibility for board certification. Candidates should also possess an active Connecticut medical license or be eligible for one, and bilingual English/Spanish skills are highly preferred.
