Home Health Social Worker / MSW for our Home Health location in Jacksonville (Duval, St. John, Clay), FL
.

Base pay range
: $38.00/hr - $45.00/hr

• Provides social work services in accordance with the physician’s plan of treatment.
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
• Serves as a liaison with other community resources.

• Master's or Bachelor's degree in social work from a school accredited by the Council on Social Work Education.
• Currently licensed to practice as a social worker in the state of FL
  .
• Minimum of one-year Home Health experience preferred.
• Valid Driver's License.
