• Complete daily Case Management assignments (on-call assignments/weekend assignment requirements).
• Perform as a clinical resource and liaison to Case Management staff.
• Collaborate with multidisciplinary team members.

• Bachelor's Degree - Nursing

• Master's Degree - Nursing

• 3-5 Years Case Management Experience Required

• Certified Case Manager, CM (from ABOHN) Preferred
• Licensed Registered Nurse Required
