• Conduct OASIS assessments and create individualized, outcome‑driven plans of care
• Evaluate patients’ physical, emotional, and environmental needs to promote independence and safety
• Provide therapy and education to help patients reach their functional goals
• Collaborate with physicians, nurses, and other members of the care team to coordinate ongoing treatment
• Monitor progress, adjust plans as needed, and maintain accurate documentation in compliance with Medicare and agency standards
• Support patients and families with compassionate, evidence‑based care

• Current Florida Occupational Therapist (OT) license (required)
• OASIS assessment experience (required)
• Minimum of one year of home health or clinical experience preferred
• Strong communication, organization, and documentation skills
• Compassionate, professional, and dependable approach to patient care
