Occupational Therapist (OT) - Home Health - PRN role at Vital Caring Group. • Evaluate and assess patients to determine appropriateness for care
• Develop patient-specific care plans using available tools and resources, with appropriate levels and frequency of care
• Communicate with the care team, physicians, payors, patients, and families to address care needs and fulfill the patient care plan goals
• Deliver high-quality skilled care and supervise care delivered by other care team members
• Document care delivery daily in our EMR system
• Contribute to a culture of caring through individual accountability and teamwork

• Commitment to the highest level of OT licensure
• Familiar with technology; HCHB experience is a plus

• Current Occupational Therapist license and valid state driver’s license with auto liability insurance
• One year of experience as an OT in acute care, rehabilitation, or home health preferred
