Position Summary

This is a professional Career Service position responsible for serving as the Incident Management Liaison. The role includes receiving, reviewing, and entering Incident Reports into the Incident Management System (IMS), handling Abuse, Neglect, & Exploitation (ANE) reports and findings from DCF investigations, and collaborating with APD regional and state teams, providers, and Waiver Support Coordinators while upholding HIPAA requirements.

The Work You Will Do
  • Forward critical incident reports to management staff and State Office based on APD’s Incident Reporting Operating Procedure.
  • Review incident reports submitted by providers and enter them into IMS, following up as needed to obtain missing information.
  • Ensure providers submit follow‑up incident reports withinframes and that wellness visits for allegations of abuse, neglect or exploitation are conducted promptly in licensed group homes, Adult Day programs, or Supported Living arrangements.
  • Close incident reports within required time frames.
  • Review monthly DCF data downloads of abuse, neglect, and exploitation investigations, ensuring all wellness visits have been conducted for allegations not previously reported.
  • Reflect verified findings of ANE as critical incidents in IMS and report per guidelines outlined in the Incident Reporting Operating Procedure.
  • Review monthly data downloads involving ER/hospitalizations and ensure associated incident reports have been submitted.
  • Participate in meetings, conference calls, training, workshops, and other activities needed, including provider quarterly meetings and regional quarterly meetings with the QIOli>
  • Perform other duties as assigned.
Minimum Qualifications
  • High school diploma or its equivalent.
  • Experience using Microsoft Excel.
Knowledge, Skills, and Abilities
  • Computer proficiency:
    Microsoft Office Suite (Excel, Word, SharePoint, Teams, etc.) and agency‑implemented software including the Incident Management System (IMS).
  • Track and meet reporting deadlines.
  • Attention to detail.
