• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Follow infection control, emergencies, and other center policies
• Collaborate with resident’s interdisciplinary team to provide the best outcomes
• Identify and report all abnormal changes in residents’ condition to proper personnel per center policy
• Work under direct supervision in accordance with the state-specific Nurse Practice Act, facility policies and procedures, and nursing judgment.
• Deliver nursing care to patients/residents requiring long-term or rehabilitative care.
• Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response.
• Direct and supervise caregiven by other nursing personnel in selected situations, in accordance with state-specific regulations.
• Maintain knowledge of necessary documentation requirements.
• Maintain knowledge of equipment set-up, maintenance and use (e.g., monitors, infusion devices, drain devices).
• Maintain confidentiality and patient/resident rights, regarding all patient/resident personnel information.

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two Years prior nursing experience preferred
