This position is based in Jacksonville, FL, serving patients in ALF's in Duval County.

• Conduct in-home OASIS assessments, evaluations, and develop personalized, goal-driven treatment plans.
• Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs).
• Collaborate with patients, caregivers, and a clinical team to deliver exceptional care.
• Utilize dictation services to streamline documentation and maximize time for patient care.
• Deliver hands-on nursing care, including:
  Acute & chronic disease management;
  Medication management;
  Post-surgical rehabilitation support;
  Pain management;
  Wound care.

• Unincumbered Florida Registered Nurse license (required)
• Valid driver’s license, auto insurance, reliable transportation (required)
• Home health care experience with OASIS documentation proficiency (required)

At Concierge Home Care, RNs work autonomously within a collaborative clinical team. We offer schedules to meet your needs, including weekend options.

• Weekend Full-Time (Per Visit Rate)
• Every-Weekend (Friday-Sunday) or (Saturday - Tuesday) 3 or 4 days required;
  No nights or 12-hour shifts required
