Telos Health Systems is seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to provide behavioral health services to the geriatric population in Skilled Nursing Facilities (SNFs) and/or Assisted Living Facilities (ALFs) throughout the Jacksonville, Florida area.

Telos Health Systems provides Psychiatric Mental Health Nurse Practitioners and General Medicine Nurse Practitioners to improve residents' quality of life and overall well-being while helping reduce avoidable hospitalizations and improve continuity of care.
What You Will DoAs a PMHNP, you will provide psychiatric and behavioral health services to residents in Skilled Nursing and/or Assisted Living settings. You will collaborate with facility nursing staff, physicians, social workers, therapists, families, and other members of the interdisciplinary care team to support the mental and emotional well-being of residents.
Responsibilities- Conduct comprehensive psychiatric and behavioral health assessments.
- Evaluate, diagnose, and treat a broad range of psychiatric and behavioral health conditions.
- Develop individualized treatment plans based on each resident's clinical needs.
- Prescribe and manage psychiatric medications in accordance with applicable laws, regulations, and clinical standards.
- Monitor medication effectiveness, side effects, and clinical response, making adjustments as appropriate.
- Evaluate residents experiencing changes in mood, behavior, cognition, or mental status.
- Provide ongoing follow-up and management of acute and chronic psychiatric conditions.
- Address behavioral health needs commonly associated with the geriatric population, including depression, anxiety, dementia-related behaviors, psychosis, and other mental health conditions.
- Collaborate with attending physicians, medical directors, nursing staff, and other healthcare professionals regarding resident care.
- Participate in interdisciplinary care planning and coordination.
- Communicate effectively with residents, families, caregivers, and facility staff.
- Facilitate referrals to appropriate behavioral health or medical specialists when needed.
- Provide appropriate documentation and CPT coding in accordance with clinical and regulatory requirements.
- Participate in medication management and psychiatric follow-up visits.
- Assist with advanced care planning and goals-of-care discussions when appropriate.
- Maintain timely and accurate clinical documentation.
- Support efforts to improve resident outcomes and reduce avoidable hospitalizations.
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active Florida APRN license or eligibility for Florida licensure.
- Active DEA registration.
- Previous experience in nursing, psychiatric, behavioral health, or another relevant medical field.
- Minimum of one year of experience in a Skilled Nursing, Assisted Living, geriatric, psychiatric, or related setting preferred.
- Experience working with the geriatric population preferred.
- Ability to build rapport with residents and their loved ones.
