Registered Nurse (RN) - Home Health | (Duval, Nassau, Clay)
Join a patient-focused home health team serving the Jacksonville, FL community. We're seeking a compassionate, skilled RN who thrives in an independent setting and is passionate about delivering high-quality, personalized care in patients' homes.
In this role, you'll assess patient conditions, develop and manage individualized care plans, and provide hands-on clinical support. You'll collaborate closely with physicians and interdisciplinary teams while educating patients and families on wellness, prevention, and recovery. Strong clinical judgment, attention to detail, and a calm, patient-centered approach are essential.
What You'll Do:
- Monitor, assess, and document patient conditions
- Administer medications and treatments per physician orders
- Perform routine procedures and maintain accurate records
- Educate patients and caregivers on care plans and health management
- Coordinate with care teams to ensure optimal outcomes
What You Bring:
- Active RN license (Florida)
- Minimum 2 years of home health or acute care experience
- Must have Oasis experience
- EMR experience (HCHB or similar preferred)
- Strong communication, critical thinking, and organizational skills
- Reliable transportation (mileage reimbursement available)
