Our Client is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join their team. As an LPN in home care, you will provide high-quality nursing services to patients in their homes, ensuring their comfort, safety, and well-being. This role requires excellent clinical skills, strong communication, and a genuine commitment to improving patients' quality of life through personalized care.

Responsibilities

-Provide skilled nursing care to patients in their homes, including administering medications, monitoring vital signs, and managing chronic conditions

-Collaborate with healthcare professionals and family members to develop and implement individualized care plans

-Document patient care accurately and timely, maintaining all necessary records and reports

-Educate patients and families on health management, medication usage, and post-treatment procedures

-Ensure compliance with healthcare regulations and company policies

-Respond to patient emergencies and communicate changes in health status to supervising nurses or physicians

Requirements

-Valid LPN license in the state of Florida

-Previous experience in home care or a related nursing field a plus but not required.

-Strong clinical assessment and patient care skills

-Excellent communication and interpersonal abilities

-Ability to work independently and manage time effectively

Reliable transportation to travel to patient homes
