Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based, non-prescribing role emphasizes education, gap closure, and collaboration with primary care providers to improve patient outcomes.

We offer a $10,000 sign-on bonus for external candidates, flexible scheduling (8am–8pm 7 days), no on-call, weekends or holidays, and a total cash compensation range of $109,500–$164,000 plus bonus
