Role DescriptionThis is a full‑time, on‑site position for an Aftercare Case Manager based in Jacksonville, FL. The Aftercare Case Manager will be responsible for providing holistic case‐management support to individuals transitioning from homelessness to sustainable living. Daily responsibilities include assessing client needs, creating personalized care plans, coordinating resources, and following up to ensure success. The role also involves collaboration with service providers to address housing, employment, health, and other support service needs.
Qualifications- Experience in case management, client support, and social services
- Strong communication and interpersonal skills to engage effectively with clients and stakeholders
- Ability to manage, organize, and prioritize multiple tasks and caseloads efficiently
- Knowledge of local community resources and support networks for housing, employment, and healthcare
- Empathy, patience, and a commitment to aiding underserved populations
- Proficiency in maintaining accurate client records and documentation
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred)
- Experience working with homeless populations or vulnerable communities is a plus
