Wolfson Children's Hospital, part of Baptist Health in Jacksonville, is hiring a Resource Navigator to join The Players Center in Downtown. This full-time role supports patients and families by identifying social determinants of health and connecting them with community resources.
You will provide information and follow up to ensure connections are beneficial and that resources meet needs, documenting activities and progress as part of a collaborative care team.
