Pay: USD $37.43/hr – $58.02/hr schedule:
Part Time, Days.
Sign-On Bonus:
Up to $5,000.
Location:
Jacksonville, IL.
- Direct, supervise, evaluate and provide physical therapy services to patients.
- Observe, record, and report patient progress; communicate with physicians as needed.
- Develop, review, and adjust treatment plans in collaboration with the physician.
- Instruct, teach, and supervise patients regarding physical therapy procedures; involve family and caregivers as appropriate.
- May supervise physical therapist assistants, rehabilitation aides, volunteers, or students in patient care techniques and needs.
- Submit appropriate written reports on patient progress at designated intervals; maintain accurate administrative and billing documentation.
- Assist in determining patient needs and developing the total plan of care; ensure care aligns with referrals.
- Adhere to standards of care, ethics, and facility policies; meet productivity and utilization expectations.
- Maintain department policies, procedures, quality assurance, safety, environmental and infection control programs.
- Assist in care and maintenance of department equipment and supplies.
- Perform other related work as required.
- Licensure/Certification/Registry: Illinois licensed/licensure eligible as a Physical Therapist.
- Certified in CPR or complete certification within 30 days of employment.

- Physical Requirements and Environment:- Ability to work professionally in a team environment with patients, management, co-workers and physicians.
- Strong organizational and verbal communication skills.
- Ability to multi-task effectively.
- General office and healthcare setting with exposure to infectious disease; potential exposure to blood borne pathogens and body fluids.
- Requires moderately heavy physical exertion for patient transfers and assistance with ambulation therapy.
- Frequent standing, walking, reaching, stooping, kneeling, crouching or crawling; occasional sitting.
- Occasionally lift and/or move over 100 pounds.
- Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus.
