Pyramid Healthcare is seeking a compassionate and motivated Substance Abuse Counselor to join our Assertive Community Treatment Team (ACTT) in Jacksonville, NC
. This role provides direct outpatient Substance Use Disorder (SUD) services to individuals with serious mental illness and co-occurring substance use disorders in community-based settings.

Through our Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), Medication-Assisted Treatment (MAT), Cross-Area Services, ACTT, and other specialty programs

• In-house licensure supervision
• Company vehicle and phone provided (mileage reimbursement included)
• Comprehensive on-the-job training
• Flexible scheduling
• Crisis phone rotation: 1 weekday and rotating weekends
• Supportive, mission-driven multidisciplinary team

• Provide community-based substance use treatment services to ACTT clients in homes and local community settings
• Conduct individual, group and family therapy sessions
• Complete LOCAs, pre-authorizations, and clinical reviews in a timely manner
• Develop and maintain treatment plans, NCTOPPs, and discharge/aftercare plans aligned with client goals
• Maintain ongoing communication with families, referral sources, and support systems
• Deliver interventions for individuals with severe and persistent mental health conditions and substance use disorders
• Participate in on-call and emergency service rotation, including crisis planning, after-hours assessments, and hospital diversion
• Attend treatment team meetings and required staff trainings
• Perform additional duties as assigned

• LCAS(A) or CADC required
• Ability to obtain required certification/licensure within 90 days of hire (if applicable)
• Qualified Professional (QP) status per 10A NCAC 27G.0104
• Valid North Carolina driver’s license
• Ability and willingness to travel locally within the Jacksonville area
