Overview
Licensed Placement Support Coordinator (Day shift) role at Carebridge EAP. This field-based, statewide position supports the North Carolina Department of Health and Human Services (DHHS) and the Foster Care team to place children in appropriate foster homes, with an emphasis on integrated physical and behavioral health care.
Location & TravelLocation:
Statewide in North Carolina. Travel within your assigned DSS Region is required. When not in the field, work virtually from home. These roles are statewide field-based and require interaction with patients, members, or providers in person four to five days per week. Hybrid/virtual work policies apply; candidates outside a reasonable commuting distance may not be considered unless an accommodation is granted by law.
- Perform case management telephonically and/or through home visits within the scope of licensure for programs like Foster Care.
- Assess and identify appropriate foster home placements considering each child’s needs and circumstances.
- Develop comprehensive care plans to address objectives identified during assessments.
- Facilitate pre-placement visits and introductions to help children and families transition smoothly.
- Coordinate with internal and external resources to meet integrated physical and behavioral health care needs.
- Maintain strong relationships with foster families, providing ongoing support throughout the placement process.
- Support members’ access to quality, cost-effective care and modify plans as needed.
- Maintain accurate records of placements and communications in alignment with organizational and legal requirements.
- Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the State of North Carolina.
- Requires a MS/MA in social work, counseling, or a related behavioral health field, or a nursing degree.
- Requires three (3) years of experience in care management, case management, or care coordination for individuals served by the child welfare system.
- Previous experience in foster care, social work, or related fields.
- Strong understanding of the foster care system and child welfare policies.
- Excellent interpersonal and communication skills; ability to work collaboratively.
- Proficiency in Microsoft Office Suite and experience with child welfare databases.
- Empathy, compassion, and a strong commitment to child welfare.
