Performs difficult professional work providing protective social work services to clients in the Child Protective Services, In-Home Services, or Permanency Planning (adoption/foster care) units of the Department of Social Services, and related work as apparent or assigned. This social worker is responsible for scheduling the Child and Family Team meeting with the family's assigned social worker, preparing the meeting location, preparing meeting summaries for distribution to all meeting participants.

Coordinates and facilitates child and family team meetings and Permanency Planning Reviews, and promotes communication between agency staff, families and their identified support persons and service providers.
Introduces and explains the meeting goals, process and ground rules to meeting participants and guides discussion of the family's strengths, needs and reason for agency involvement and steers the meeting participants toward group consensus regarding recommendations for the family.
Collaborates with the team to create or revise safety, service, and placement plans. Ensures plans reflect the strengths, needs, and preferences of the family.
Builds trust and rapport with families and community partners to encourage active participation.
Document meeting outcomes and decisions. Maintain accurate records in case management systems. Share meeting summaries and plans with all stakeholders.
Manage conflict and emotional dynamics in meetings. Help families and team members navigate disagreements constructively.
Provides information to and confers with schools, natural parents, and resource parents on habilitation plans, behavior management, and other problem areas or needs.
Conducts interviews and home visits to verify information, conduct assessments, and evaluate service/treatment plans.
Facilitates or develops community or service resources and/or develop volunteer programs.
Performs assigned functions as a Disaster Ready Team (DRT) member.
Performs related work as required.
Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem. Ability to establish and maintain effective working relationships with administrative superiors members of case load and their families, and with civics medical, social and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express ideas clearly and concisely and to plan and execute work.
- Bachelor's degree required in social work or related field
- Master's degree preferred
- Valid driver's license
