UPMC Chautauqua is seeking a Full-Time LPN to join our team in our Residential Treatment unit. This position offers 36 hours/week on Night Shift with potential for career growth and advancement.

About the Unit

Our Residential Treatment unit is an Ambulatory Residential Addiction unit certified by the State of New York’s Office of Addiction Services and Supports (OASAS) and is designed to support individuals diagnosed with a substance use disorder. The 20‑bed program includes dining and kitchen areas, a group room, TV and computer rooms, an art therapy room, gym access, and indoor recreational spaces.

We use evidence‑based programming combined with vocational and wellness therapies to create a person‑centered environment. The staffing team includes Psychiatrists, Physician Assistants, Nurses (RNs and LPNs), Counselors, Milieu Therapists, Supportive Employment Specialists, Psych Associates, Personal Care Technicians, and Peer Specialists who meet the various needs of clients throughout treatment.

Sign‑On Bonus Details
  • $12,500 for one or more years of LPN experience.

Responsibilities
  • The LPN functions as a member of the health‑care team, exercising sound nursing judgment based on preparation, knowledge, and experience.
  • The LPN participates in the planning, implementation, and evaluation of nursing care using focused assessment.
  • The LPN communicates with a licensed professional nurse and the patient’s health‑care team to seek guidance when the patient’s care needs exceed the LPN’s scope or when the patient’s condition changes significantly.
  • The LPN obtains instruction and supervision when implementing new or unfamiliar nursing practices or procedures, following UPMC policies, the UPMC LPN Practice Guidelines, and the State Practice Act.
  • The LPN formulates daily goals and a plan of care for patients, involving the patient as a partner and considering individual needs holistically.
  • The LPN maintains comprehensive documentation to promote communication among caregivers and supports continuous learning through participation in organizational programs.
  • The LPN develops and maintains productive working relationships with peers and management, demonstrating accountability, enthusiasm, and commitment to patient care.
Required
  • Successful graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing.
  • Competency in providing care appropriate to the ages of patients served, with knowledge of growth and development principles.
  • Experience in health care preferred; LPN experience not required.
  • Ability to work productively within a complex environment, handle multiple priorities, and operate specialized equipment.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.
  • Computer skills required to utilize the electronic health record.
Licensure, Certifications, and Clearances
  • LPN must maintain state licensure or be covered by a licensure compact agreement where the facility is located.
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR).
  • Must complete licensure examination within four months of hire or within one year of graduation.
  • New York SCR application due within first week of hire.
  • Comprehensive Crisis Management (CCM) training required within 60 days of hire and annually thereafter.
  • New York State Fingerprint Check; provisional work may be granted for 30 days.
  • NY State Fingerprint Check OASAS (NYFP2), NYS Staff Exclusion List (NYSEL), and NY Statewide Central Register of Child Abuse (NYSCR).
