Transitions At Home, is currently seeking to hire a Full-Time Physical Therapist. We are seeking a compassionate and driven Physical Therapist to provide personalized, one‑on‑one care in patients’ homes—helping them regain strength, mobility, and independence. An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

• 8:00am – 4:30pm
• Monday through Friday

• Active Wisconsin Physical Therapist License
• Two (2) years’ experience preferred.
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
