Behavioral Health Case Manager, Child and Adolescent Day Treatment - FT

OverviewThe Behavioral Health Case Manager MS works with the multidisciplinary team to provide care for patients admitted to inpatient behavioral health, mental health, addictions, and child/adolescent day treatment programs. The role involves disposition planning, care planning, group psychoeducation, and consults in the ED and on medical units under the supervision of a psychiatrist.
Responsibilities- Consults and coordinates care of patients with physicians/psychiatrists, RN, social workers/therapists, and other agencies.
- Assesses treatment planning and progress.
- Prepares treatment plans and goals for patients.
- Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
- Provides psycho educational groups.
- Arranges for disposition and follow‑up recommendations.
- Master’s degree in a mental health or behavioral health field.
- Experience or field study experience in a mental health setting.
- Working toward licensure is preferred.
- TACT certified within 90 days after hire.
- Approved CPR/BLS preferred.
