Performs all functions of pharmaceutical care including medication and IV orders. Evaluates for appropriate drug, dose, allergies, interactions, and incompatibilities. Monitors levels of therapeutic drugs. Performs and supervises computer data entry related to patient profile. Monitors all medication dispensing for accuracy. Supervises filling of all prescriptions and provides appropriate patient counseling. Collects and analyzes data for process improvement projects. Performs in-service to community and support groups.

Performs other duties as assigned. May be required to work alternate shifts. Performs other duties as assigned.

• Maintains professionalism and conducts self as a role model, serving as a resource for all health system partners.
• Is an expert staff pharmacist who relies on skills of comprehensive knowledge and experience.
• Maintains practice by ongoing professional literature review, continuing education and self-evaluation.

Graduate of an accredited Pharmacy Program

Hospital Pharmacy experience or residency preferred

Wisconsin Registered Pharmacist License or eligible

Excellent interpretive/analytical skills for pharmaceutical applications

Strong accuracy skills

Computer experience

Work requires a moderate level of independent judgment along with good interpretative and interpersonal skills with justification and accountability to Supervisor, Manager, and Director.

This position supervises technicians. This position is involved in the recruitment and orientation process and provides performance feedback and input for performance evaluation purposes. May implement coaching, counseling, and corrective action when needed.
